Transition Assistant- SEND FTC

Are you passionate about supporting young people with additional needs to achieve their full potential? Moulton College is seeking a dedicated and enthusiastic Transition Assistant - SEND to play a key role in helping students with disabilities successfully transition into college life and beyond.

Working closely with students, families, Local Authorities, and college staff, you will deliver a bespoke transition journey tailored to each learner's individual needs.

From the moment a prospective student expresses an interest in joining Moulton College through to becoming settled on the most appropriate course, you will provide guidance, encouragement, and practical support every step of the way.

Please note this position is a fixed-term contract until July 2027.

Interviews for this role will be taking place week commencing 26 October 2026.

  • Deliver bespoke transition programmes for students with SEND, tailored to individual needs, Education, Health and Care Plans (EHCPs), and agreed funding arrangements.
  • Liaise closely with students, parents/carers, and current education settings to ensure a smooth and successful transition into college and then into employment or long-term support.
  • Support prospective students in exploring course options and identifying the most appropriate learning pathway to meet their aspirations and support needs.
  • Act as an advocate for students with SEND, promoting independence, confidence, and positive outcomes throughout their educational journey.

A Driving License and access to your own vehicle is essential as some travel within the local area is also required.
